Why Karthus gets an S
Requiem applies pressure to the whole map. Every low recall, every skirmish two lanes away, and every greedy objective attempt happens under a global execute, which the enemy has to account for in decisions he never physically attends.
The itemization point in the verdict matters just as much. Every hourglass and spell shield bought to answer him is an item that is not killing his carries.
Death Defied removes the normal ending. He keeps casting for a few seconds after dying, so the enemy's best case still gives up a full rotation of damage.
Karthus has to die in the right place
The passive is only worth something if he dies within range of the fight, and the global is only worth something against targets low enough to finish. A Requiem cast into full health bars for the scoreboard is a wasted cooldown.
